Text Box: Note: Examples how to write source of information, ILO Code or HSE guidance with references are below ….

“Managing risks and risk assessment at work” which provide guide about detail on managing risks at workplace and approach is similar to five step approach. https://www.hse.gov.uk/simple-health-safety/risk/index.htm?utm_source =hse.gov.uk&utm_medium=refferal&utm_campaign=risk&utm_content=home-page-info

 “Managing plastics safely” https://www.hse.gov.uk/plastics/basics.htm and “Safety at Injection Moulding Machines” https://www.hse.gov.uk/pubns/ppis4.pdf .

Part-2 Risk Assessment

Organization Name: _______________________________

Date of Assessment: ______________________________

Scope of Risk Assessment: (particular area or departments which you choose  for risk assessment)

In table, 12-13 different hazards that are taken from at least5-6 different hazards categories

Column-1 Hazard category and hazard (concisely)

Column-2 Who might be harmed, what harm and how? (Some description)

Column-3 What are you already doing? (Already controls/precautions in place)

Column-4 What further controls/actions are required?

Column-5 Timescales for completion of each control separately.

Text Box: Note-1
When you complete your risk assessment you will see that “Column-3” and “Column-4” work together. If you are already doing enough to control the risk, there will be a lot of controls listed in “Column-3” but very few in “Column-4”). On the other-hand, if you have very few controls in place, “Column-3” will contain very little and “Column-4” will contain a lot more. Both of these scenarios are fine because they are realistic. (About 8-10 controls are required for both Column-3& Column-4 altogether).
Column-6 Responsible person for each action/control, separately (Designation required)

Text Box: Note-2
Hazard categories may be;
Noise; vibration; radiation; mental ill-health; violence at work; substance abuse at work
